Instructions For Distributing Your EAP Summaries

Within 5 days of an event check the accuracy of the information in this EAP Summary and make any required updates by making appropriate changes in your Team Profile. Once the summary is complete use the Stare button in the top right corner of the page to send  this document to appropriate home and visiting personnel.  Enter the email addresses of those you would like to receive a link to this document, then select a date at which time the link will expire. This link will allow recipients to open the responsive EAP Summary document and take full advantage of all its features on any computer or internet capable mobile device, including the auto-dial and mapping features. Have all appropriate personnel review this responsive document during the event’s Medical Time Out.

Instruction for Recipients

Laminate the printed version of  the Team EAP Summary and post conspicuously throughout the event venue.

Instructions For Using This Summary

Upon identifying an at risk athlete/condition Share this overview with all appropriate medical team members using the Share feature accessed via the Share button at the top of this page.  Should it become necessary to enact this protocol you may use the Enhanced Vital Sign Trending Report to track and monitor vital signs to aid in patient care and assessment.  After completing the Vital Signs Trending Report click the Submit button at the bottom of the form.  The information will be saved on your Hub Page under the  Sentinel Event tab

Printing / Posting Instructions

Set your printer to 2-sided printing, and print pages 2 and 3 of this document.  Laminate the printed version of this summary and post conspicuously throughout the event venue.
