Instructions For Creating Your Pre-Event Medical Team Meeting

Begin by selecting the Revise button at the top right of the page to open the document editor.  With the document editor open provide a name for the new document you are about to create.  Next, scroll through the document and complete all required fields.  Once you completed all necessary fields click on the Submit button at the bottom of the page.  Your new Medical Time Out document is now available from your Team Hub page under the Custom EAP tab.  The original Medical Time Out Template remains available for future revisions by clicking on the Medical Time Out link from your Team Hub page.

Printing The Pre=Event Medical Team Meeting Checklist

After creating your event specific medical time out using the Revise function described above, open your new document via the Custom EAP tab on your Team Hub page.  Click the PDF View from the top right menu options.  Set your printer to print 2-sided and print only the second page of the document.

Documentation

Once you have completed the medical time out exercise ensure that all in attendance sign the medical time out document.  The completed Pre-Event Medical Team Meeting Checklist should then be signed by the lead reviewer and a witness.  The completed and signed document should then be submitted to league officials in accordance with league requirements and then filed appropriately.
